To request access to the CCHC FollowMyHealth Patient Portal, please send email to patientportalrequest@cchealthcare.com and provide the following information:
- First Name
- Last Name
- Suffix
- Date of Birth
- Email Address
- Last 4 Digits of SSN
- CCHC Primary Provider

- Click the registration link in the email you receive from noreply@followmyhealth.com or by clicking on the CCHC Follow My Health Patient Portal link on our Website cchchealthcare.com
- Create your portal login using a unique username and password.
- Enter the invitation code provided by your medical organization and click “Agree” to the release of information.
After successful signup, you will receive two emails:
- one welcoming you to the portal
- one confirming your connections with our organization
