~ CCHC knows you; we have your records, and will try to see you right away.
~ CCHC can help determine which office is best to treat you to avoid unnecessary trips to the Emergency Room.
~ CCHC has high quality acute care at a lower cost when you do not need a hospital.
HOW TO REACH US?
Mon-Fri 8am-5pm ~ Call your doctor’s office. If you have an acute illness, ask to speak with a nurse. Clearly state you need to speak to a nurse and do not leave a message.
After Hours ~ Call our Nurse Triage Line at 252-636-NURS (6877). Our nurses have access to your medical record and your physicians. They are there to help.
WHERE DO I GO AND WHO WILL I SEE?
~Your Doctor’s Office ~ Your care team will try to get you in for a same day appointment. Your care team includes physicians, physician assistants, nurse practitioners, care managers, nurses, and other clinical staff. Our goal is to address your urgent care needs at one of our specialized clinics the same day you call.
~ CCHC Urgent Care ~ Open 8am-7:30pm weekdays; 9am-4pm Saturdays and holidays (closed on Thanksgiving and Christmas), and 1pm-6pm Sundays. Urgent Care has a “phone ahead, wait at home” program that minimizes wait times. Visit us online or call 252-638-2273.
~ CCHC Extended Care ~ This clinic is staffed by Emergency Medicine physicians, Hospitalists, and Cardiologists with equipment that can evaluate and treat more acutely ill patients who traditionally have gone to the hospital. The Extended Care Clinic is by referral only, meaning that your regular care team must call first to make sure this clinic is appropriate for your situation.
