Eligibility for COVID-19 Booster Shots

CCHC offers COVID-19 vaccines at most locations. If you are a CCHC patient, your doctor’s office will text you a link to register for an appointment. If you do not receive texts, please contact your doctor’s office to make an appointment for your vaccine today!

  • The approved populations for boosters are:
    • 65 years and older
    • Age 18+ who live in long term care settings
    • Age 18+ who work in high risk settings
    • Age 18+ who live in high risk settings
    • Age 18+ who have underlying medical conditions (per CDC’s list)
      • Cancer
      • Chronic kidney disease
      • Chronic liver disease
      • Chronic lung disease
      • Dementia or other neurological/mental conditions
      • Diabetes
      • Downs Syndrome
      • Heart Conditions
      • Immunocompromised State
      • Overweight and Obesity
      • Pregnancy
      • Sickle cell and thalassemia
      • Smoking and substance use disorder
      • Transplant
      • Stroke
      • TB

Keep Scheduled Appointments, Take Medications as Prescribed

You should not avoid your regularly scheduled appointments out of concerns for COVID-19 unless otherwise instructed by your physician. If you are concerned about your symptoms, call your doctor before your appointment.

Call us First

If you experience symptoms, call your doctor and explain how you feel over the phone before heading to a doctor’s office, urgent care or the emergency room. Your doctor’s office or urgent care center will give you special instructions and prepare for your arrival so they can provide you the care you need while protecting others.

Your doctor will determine if you need to be tested for COVID-19 and what to do next. When you call ahead, you may be asked to wait at your car so a staff member can bring you a mask and escort you into the clinic. This will help prevent the spread of illness in the community.

Virtual Medical Visit

Virtual visits are now available at CCHC. A virtual visit will allow you to connect with our clinicians from any mobile device or computer with a webcam. Please call your primary care office or urgent care to see if this service is right for you.
